by all prospective customers (retail and wholesale):
- Company Name
- Contact Name
- Ship To Address
- Bill To Address
- Phone Number
- Fax Number
- Dun & Bradstreet Number (if available)
- Tax Exempt status with photocopy of certification
- Number of years in business
- Organization Type (corporation, sole proprietorship, co-partnership, limited partnership, etc.)
- Three major open credit supplier references preferrably in publishing industry (include name of company, address, phone number, and account number.)

letterhead via mail or fax along with an opening order, or
